Once your reservation details have been confirmed over the phone, please fill out the "Final Reservation Confirmation Form" below.
*This form will confirm your reservation, and we will charge approximately 50% of the application amount (tax included) for each meal plan as a minimum guaranteed fee.

- An additional 10% service charge will be charged.
- There may be some dates where we cannot accommodate your request, such as due to reservation status or event dates.
- The menu may change depending on availability.
- If you wish to cancel for personal reasons after submitting your reservation using the Final Reservation Confirmation Form, please be sure to contact Ristorante AO Zushi Marina (0467-25-0480).
- Please note that if you cancel before the 8th business day, we will refund the minimum guaranteed fee that you prepaid, minus the transaction fee.
- If we decide to suspend operations due to unavoidable circumstances (such as natural disasters or social conditions), we will refund all fees excluding payment fees.
